Registration of Vital Events that affects the Civil Status of an Individual

Certificate of Live Birth (COLB)

Schedule of availability of Service:

Monday to Friday             –           8:00 A.M. – 5:00 P.M. (without noon break)

On Time: 30 days from date of birth

Fee/s: NO FEE

 Requirement/s:

  • Four copies of accomplished Certificate of Live Birth
  • In case of an illegitimate child and should the father will acknowledge/admit the paternity of the child, he must personally appear to the MCR Office to affix his signature on the Affidavit at the back of the COLB form.
  • Current year’s CEDULA or Community Tax Certificate (CTC) of acknowledging parent.

STEPS/ PROCESS

1 Fill-out information sheet at the MCR Office (birth delivered thru traditional midwife or hilot) COLB and requirements are examined if correct and complete. Client is interviewed for verification of information provided in the submitted form. Require informant to affix signature on the COLB form. 2 COLB and requirements are presented for registration (birth in municipal hospital, lying-ins and other institution allowed to accomplished the COLB) COLB and requirements are examined if correct and complete. Client is interviewed for verification of information provided in the submitted form. Require informant to affix signature on the COLB form. In case of illegitimate child, the acknowledging parent/s is/are required to affix signature in the presence of the MCR.

Assign a registry number and submit to the MCR for signature of the COLB form.

Certificate of Live Birth (COLB)

Delayed: Beyond 30 days from date of birth

Fee/s: P 100.00

Requirement/s:

  • Four copies of accomplished Certificate of Live Birth
  • In case of an illegitimate child and should the father will acknowledge/admit the paternity of the child, he must personally appear to the MCR Office to affix his signature on the Affidavit at the back of the COLB form.
  • Current year’s CEDULA or Community Tax Certificate (CTC) of acknowledging parent/s.

Note: Child born before the New Family Code of the Philippines, both parents will acknowledge.

  • Negative Result from PSA
  • Marriage Contract (if married)
  • Affidavit of two disinterested persons
  • At least two of the documents listed below;
    • Baptismal Certificate
    • School Record/Form 137/Transcript of Record
    • Voter’s Registration Record/Certification
    • NBI Clearance
    • SSS Membership (E-1)
    • Employment Record
    • Withholding Tax certificate
    • Passport
    • Driver’s License
  • 2 valid IDs with date and place of birth

Certificate of Death (COD)

Schedule of availability of Service:

                        Fee/s: P 25.00 – Burial Fee

 Requirement/s:

  • Four copies of accomplished Certificate of Death with signature of the embalmer at the COD form.

Check the signature of the Municipal Health Officer (MHO), Chief of Hospital and other Physician as well as the signature of the Embalmer. Certificate of Death (COD)

  • Four copies of accomplished Certificate of Death with signature of the embalmer at the COD form.
  • Current year’s CEDULA or Community Tax Certificate (CTC) of the informant
  • Authenticated copy of the certificate of burial, cremation, or of other means of corpse disposal
  • Negative Result from PSA

Certificate of Marriage (COM)

Schedule of availability of Service:

Monday to Friday        –         8:00 A.M. – 5:00 P.M. (without noon break)

                    On time: marriage with marriage license (15 days from date of marriage)

                        Marriage with exceptional character, ex. Art. 34 (30 days from the date of marriage)

                        Fee/s: NO FEE

Requirement/s:

  • Four copies of accomplished Certificate of Marriage
  • In case of marriage with exceptional character, the solemnizing officer shall affix his signature on the Affidavit at the back of the COM form. This affidavit must be notarized or signed by the administering officer.

  • Four copies of accomplished Certificate of Marriage
  • In case of marriage with exceptional character, the solemnizing officer shall affix his signature on the Affidavit at the back of the COM form. This affidavit must be notarized or signed by the administering officer.
  • Current year’s CEDULA or Community Tax Certificate (CTC) of the applicant
  • Affidavit of delayed registration
  • Certified copy of the application for marriage license.
  • Negative Result from PSA
